Who is to blame for the oil spill?

Some say Obama has been slacking in his leadership of the country. "Is anyone driving this ship?" asks David Frum, CNN. There are also those making the point that the Feds had a response plan put in place in 1994 but failed to instigate it.

Main organizations involved are playing the blame game including BP America, Transocean, and Halliburton. Who should be responsible for cleanup costs? Ultimately, the truth must be discovered in order to reach a solution. All organizations involved could be sued for costs of repairs by business that have been affected by the economic fallout caused by this catastrophe.
BP says it's Transocean's fault because there was a malfunction in the shutoff valve. But Transocean says that VP sets the specifications! Transocean also says that Halliburton is responsible for the casing so they should be held responsible. Halliburton says that they did the job according to BP's specifications, so it there is a problem, it is with BP's specifications and Halliburton should not be held responsible.
